Hi <insert parent/guardian name>,

Thank you for submitting the COVID form for <insert player name>. The COVID Task Force has reviewed this information and has determined the following course of action.

Based on the information provided, with the player's confirmed Positive COVID test, per the <insert association name> policy this player will need to suspend play for 10 days. This is in effect from the date of the first symptoms (if known) or the test date (if no symptoms, which was Wednesday 11/11, resulting in a return to play on 11/22/20. Return to play is contingent on the player showing no symptoms as well.

In addition, if any parent is a coach on the team, they too will need to suspend participation with this team. We will also notify the team's head coach to make sure they are aware of this absence.

If you have any further questions, please feel free to contact me directly. The COVID Task Force is able to further review any details as they arise.

 Thanks,

Message to Coach

Hi Coach,

Reaching out to inform you that <insert player name> will have play suspended through 11/25/20, as the player was in close contact with a confirmed test on 11/11. The team is not impacted as they were not in contact with a confirmed positive test within 48 hours for this instance, as the player hasn’t been confirmed positive (and the team is 2 steps removed from the positive case).

Please let me know if you have any questions.

As always, keep this information confidential.

Thanks,

Message to MDH (for Positive Cases)

To whom it may concern,

I am currently representing <insert Association name>, with regards to our COVID Task Force. A positive COVID test was reported to our task force and below are the details we are passing along.

  • Sample Team Boys 4th Grade Blue
  • First date of symptoms 11/12
  • Test date of 11/14
  • Player will need to be out 10 days until 11/23 (play suspended through 11/22)
  • Symptoms will also need to be improving
  • As for the team, last contact was 11/9, so no impact to the team as it was prior to the 48 hour window from first symptoms
  • Our association has a stated COVID policy, adherence to RSVP tracking for all practice/games and has the appropriate forms available to complete when cases arise.

At this time, we do not have any questions regarding this case, but if you feel there are other insights we should be aware of please let us know.

Thanks,

Message to Team (Close Contact)

Good morning,

The Covid-19 Task Force has learned that a player on your daughter’s team has a confirmed positive Covid-19 test. Due to this close contact, this team will need to suspend play for 7-10 days (7 with a negative test, 10 with no test). The suspension of play is effective from 12/1 (last date of contact), with a return to play 12/9 (with a negative test) or 12/12 (with no test).

Please know that we will always put the health and safety of the <insert association name> players and families above all else. To help us with that, I want to take this opportunity to remind you of our COVID policy, requiring players to submit a form for either (1) Positive COVID test, (2) Associated Symptoms or (3) Close Contact. With associated symptoms, if the player has any of the listed symptoms, it is important to submit a form. The COVID task force will utilize this information to determine potential impact to the team and prevent further spread amongst the association. Without consistent reporting, the opportunity for spread increases. The end goal is to prolong our season and ensure we minimize impact to games/practices.

Thank you for your understanding of this situation. Please let me know if you have any questions or need additional information.

Thanks,
